Our Patients:

  • Are typically older adults who live in an assisted living or residential care facility

  • Have serious chronic medical conditions that require more intensive medical management and close follow-up

  • Do not respond to an exclusively office-based plan of care

  • Have difficulty leaving their homes and getting to and from medical appointments

  • Have physical or cognitive impairments that makes travel to office-based clinics more difficult

  • Were recently discharged from a hospital, skilled nursing facility or rehabilitation facility and need a little extra help transitioning back home

  • Require an extra layer of care to help keep them living at home independently

  • Medical visits are provided by nurse practitioners (NPs) who specialize in geriatric medicine and home-centered primary care

  • We conduct comprehensive medical assessments and health screenings on all new patients

  • Provide preventive and primary care  services in the comfort of your home

  • Administer immunizations 

  • We treat and manage chronic diseases such as:

    • Heart failure 

    • Pulmonary disorders

David Hibbs, FNP-C

Nurse Practitioner and Co-Founder

David received his Bachelor of Arts in Political Science from the University of Washington in 2002.  He received his Master of Science in Nursing from Seattle University College of Nursing in 2005.  David is board certified as a family nurse practitioner by the American Academy of Nurse Practitioners.  Upon graduation he spent four years working at a community health clinic in rural central Washington providing acute and primary care to a diverse and medically under-served population.  From there, he joined the in-patient palliative care team at the Veterans Affairs hospital in Portland, Oregon, where he honed his clinical skills on the treatment and management of older adults with serious and sometimes life limiting illnesses.  David has spent the last 10 years caring for older adults in the assisted living, long term care, rehab, and post-acute care settings. He understands the unique health needs of an aging population and has focused his entire practice on caring for older adults.  David is a board member of the Oregon Geriatrics Society, a not-for-profit society that works to improve the health, independence, and quality of life for older adults.

 

David lives in Portland, Oregon, with his family and enjoys fly fishing, gardening, exploring the great Northwest, and traveling the world. 

Amy Stivers, AGNP-C

Nurse Practitioner and Co-Founder

Amy Stivers, MSN, AGNP, received her Bachelor of Science in Nursing from Brenau University in Gainesville, Georgia, in 2004. She practiced as an RN in acute care (ICU/CCU, PACU, Psychiatry). She received her Master of Science in Nursing from Emory University in Atlanta, Georgia, in 2013.  She has a dual certification in Adult-Gerontology primary care, with a background in physical medicine and rehab/post-acute care and geriatric primary care.  Her practice focuses on managing chronic disease and treating acute illness in frail elders, and is grounded in the belief that the patient is the center of the healthcare team. She assists patients and their families with understanding and identifying the most appropriate treatment plans for their goals of care, modifying those plans as goals change, and feeling honored to be able to participate in their journey through this stage of life.


Amy lives in Portland, Oregon, with her husband and dog.  She enjoys cooking, reading, travel, and playing outside.
